Below is a printable/downloadable pdf file of our service contract. Customers may print a copy and fill in the appropriate information before coming to the office to sign up for service. At this time we are not accepting service contracts through email. New customers will need a photo ID or drivers licence. Also, renters are required to complete the rental agreement before the service contract is considered complete.
All new service contracts must be brought to Douglas Water Authority office during regular working hours for verification and processing. Same day service will require processing before 3:00 PM.
