Below is a printable/downloadable pdf file of our service contract. Customers may complete the service contract and bring along with the following documents: Deed (for landowners), Rental agreement (for renters), Drivers license/photo ID to establish service. If submitting by email to office@douglaswater.org please allow up to 48 hours for service to be established. Connect fees must be paid before water will be turned on.
For same day service customer must bring documents to our office prior to 3 PM for payment and processing.
