Please print the form and complete with your information. Then you may bring the form along with payment of $15 to the office or you may mail it to us: PO Box 46, Douglas, AL 35964. Once we receive the form along with the payment, we will process and send someone out to turn the water on.
You may extend the service for an additional 2 weeks by calling the office 1 day prior to the scheduled cutoff date and we will add $15 to your account that will be added to your bill.
